Online Booking Now Available

No more waiting on hold to book an appointment....

 

SCFH is now offering online appointment booking for their existing patients. 

We understand that your time is valuable, and you may be unable to call during our clinic hours.

We are currently running a pilot program that allows existing patients at our clinic to book appointments using a secure online platform. ​

REMEMBER - If you are experiencing difficulty breathing, severe chest pains, or any other condition that could be life-threatening, please call 9-1-1 or go to your nearest emergency department.

 

For acute symptoms that appear, change, or worsen rapidly, you may call the clinic to be triaged accordingly. Please reference the list under the urgent care section for a list of other urgent matters.

As we monitor the success of this initiative, we will continue to add more doctors/nurses to this platform. Please note adjustments will be made over the course of the trial to help improve the online booking experience. We appreciate your patience.

Please use the online booking link to schedule your non urgent appointments with your family physician. These include but are not limited to;  follow-ups, medication reviews, new onset issues, cold, flu & allergy symptoms and, mental health assessments. DO NOT USE ONLINE BOOKINGS FOR CHEST PAIN, SHORTNESS OF BREATH, PREGNANCY CONCERNS, ABNORMAL BLEEDING or other serious concerns.

We now offer online booking for various appointments with our Registered Nurses, these appointments include but are not limited to;  repeat allergy injections, routine paps, suture and stitch removals, select vaccines, and blood pressure checks.

Please call the clinic directly for urgent matters such as: 

  • Accidents and falls

  • Breathing difficulties, such as asthma

  • Persistent fever or cold/flu symptoms

  • Severe sore throat or cough

  • Worsening skin rashes and infections

  • Sprains and strains

  • Urinary tract infections

  • Vomiting, diarrhea, or dehydration

Call the clinic directly at 519-925-0017, if appropriate a nurse will triage your call and book you in accordingly/ Please note we are NOT a walk-in clinic. You must be a patient at the clinic and you must have an appointment.

For medical needs not requiring in-person physical assessments or if you are unsure what you require, FHO patients may call the Clinic to speak to our staff and, if appropriate, book a phone assessment with your Physician. This cannot be done online. 

Note that the services below are NOT appropriate for phone call assessments:

  • Controlled substance renewal or prescription

  • Musculoskeletal injuries requiring an exam

  • Conditions that require a Breast, Pelvic, Genital. or Rectal exam

  • Acute or new psychiatric crises or symptoms

  • Any emergency symptoms including: Chest pain, Shortness of breath, Abdominal pain

  • New onset severe headaches

  • Allergic reaction

  • Altered state of consciousness

  • Head or neck injury

Prescription Refills

  • It is very important that you don’t wait until the last minute to request a prescription renewal. 

  • The best way to request a prescription renewal is to book an appointment well in advance of running out.

  • A phone call or faxed renewal request from the pharmacy requires at least 72 business hours for completion.

  • A prescription renewal without an appointment is not an insured service under OHIP. Starting on June 3, 2024, completion of these requests will be billed to the patient. There is no charge for requesting a refill prescription during an appointment. 

  • ​We offer individual or family plans through PatientSERV to help you save on uninsured services. If you are interested, please call the office at 519-925-0017 or ask us at your next appointment.

  • Difficulty breathing

  • Severe chest pain
